As Parent/Legal Guardian of my child, I have informed myself and my child of the applicable rules and regulations of this MTAC West LA Branch Program. I understand that any violation of the rules and regulations may result in dismissal and/or restriction from participation in future MTAC West LA Branch Programs.

I understand that parents, guardians, or parent representatives must accompany minor students during this West LA Branch event, and that students must be at a visible location of the venue, not backstage or in any other space which is not in view of their parent, guardian, or their parent representative. I understand the teacher is not considered to be the parent representative.

I also agree to indemnify and hold the MTAC West LA Branch harmless from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorney’s fees brought as a result of my participation in this MTAC West LA Branch Program and to reimburse them for any such expenses incurred.
